Missouri Medical Marijuana Certification

All new patients must print and fill out the Patient Registration and Medical History Form and bring to your appointment.

You MUST bring your medical records from the last year pertaining to the qualifying diagnosis to the appointment. If you arrive without medical records, you will be charged a no show fee and will not be able to be rescheduled until medical records are received by our office.

Patients need to refer to the states information website on medical marijuana for questions about submitting their certification and qualifying diagnoses.
